The best way to order a VisionKey or accessory is through our online order forms. There are two forms that need to be completed... due to spam we have removed the links to these forms, but please contact us for the links.

-The User Online Order Form
This is where you select the product to purchase and give us the shipping address etc.

-The Optometrist's/Optician's Information Form
We need certain measurements to ensure that VisionKey is properly set for you. You may need to, schedule an optometrist/optician visit or some will arrange to come to your home or care facility.

If you are unable to order online, you can view PDF versions of the Order Form and Optician's Form. You can also download zip compressed archives of the forms as PDF files or as Microsoft Office files (Office 97 for Windows or Office 98 for Mac required). You can then fill them in and fax or mail them to the below address.

Please note that payment must accompany the order unless accompanied by an institutional Purchase Order (P.O.). If the Order Form is faxed to us, payment must be received prior to shipment.
